Anglicare Australia Awards Nomination

The Anglicare Austalia National Awards for Excellence and Innovation recognise the outstanding contribution of member agencies, their staff and volunteers to the service of people in need, and draw attention to new, innovative approaches and programs being implemented within the Anglicare Australia network. The Awards program affirms the contribution of Anglicare agencies to the health and wellbeing of not only the individuals, but also Australian society.

Please complete the 2013 Nomination Form and attach your statement in support of the nomination, addressing each of the judging criteria listed for the category in the forms provided below.  Please send five copies of any dvd etc. or any other supporting material. If sending a hard copy, do not bind anything.

All nominations are to be submitted on the Nomination Form below, signed by the CEO of the member agency and accompanied by all required supporting documentation.  Closing date is Friday 5 July 2013.
